BLOOD PRESSURE

1. Explain the procedure to the patient.                                                                 
2. Get the thermometer, disinfect from bulb to stem.
3. Read the thermometer if in the level of 35ºC, if not shake the thermometer until it reaches to 35ºC.                                                                                            
4. Instruct the patient to open mouth and place thermometer under the patients tongue then instruct patient to close mouth.                                                 

Taking the PULSE Rate

5. While waiting for the appropriate time, palpate radial artery and count pulse for 1 minute in the proper.                                                                                        

Taking the RESPIRATORY Rate

6. Proceed to counting the pulse rate for 1 minute. Observing the proper technique, 1 hand still holding unto the radial artery. Record both RR & PR.                  

Taking the BLOOD PRESSURE

7. Apply the BP cuff with arm hyper extended.                                                     
8. Palpate brachial artery with left hand, put the stethoscope unto the ear with the earpiece the patient                                                                                           
9. Inflate cuff till pulsation disappears and add 30 mmHg                          
10. Place diaphragm bell of stethoscope over brachial artery
11. Release valve slowly, take systole, then diastole.
12. Remove cuff and record BP.                                                                            
13. Remove Oral thermometer from the mouth                                                       
14. Wipe thermometer from stem to bulb.                                                  
15. Read the temperature and record.                                                                    
16. Disinfect thermometer properly.                                                                       
17. Record TRP and Graph properly.

1. Jelaskan prosedur untuk pasien.
   2. Dapatkan termometer, disinfeksi dari bola ke batang.
   3. Baca termometer jika di tingkat 35 º C, jika tidak mengguncang termometer sampai mencapai hingga 35 º C.
   4. Instruksikan pasien untuk membuka mulut dan tempat termometer di bawah lidah pasien kemudian menginstruksikan pasien untuk menutup mulut.

Mengambil Tingkat PULSE

   5. Sambil menunggu waktu yang tepat, palpasi arteri radial dan menghitung denyut nadi selama 1 menit dalam yang tepat.

Mengambil Tingkat PERNAPASAN

   6. Lanjutkan ke menghitung denyut nadi selama 1 menit. Mengamati teknik yang tepat, 1 tangan masih memegang arteri radialis kepada. Catat baik RR & PR.

Mengambil TEKANAN DARAH

   7. Terapkan manset BP dengan lengan hiper diperpanjang.
   8. Palpasi arteri brakialis dengan tangan kiri, meletakkan stetoskop kepada telinga dengan lubang suara pasien
   9. Mengembang manset sampai denyut menghilang dan menambahkan 30 mmHg
  10. Tempatkan diafragma stetoskop bel di atas arteri brakialis
  11. Rilis katup perlahan, mengambil sistole, kemudian diastol.
  12. Hapus catatan manset dan BP.
  13. Hapus termometer oral dari mulut
  14. Usap termometer dari batang ke bohlam.
  15. Baca suhu dan merekam.
  16. Disinfeksi termometer benar.
  17. Rekam TRP dan Grafik benar.
